Last modified: November 3, 2009If you would like to request a Certified Evaluation please use this form to submit your information including your name, email, phone, & years officiated in high school. Please also mark which dates would be your preferred evaluation dates and the list of locations and times for each. We need multiple options to try and accommodate our Evaluators schedules. We will reply to your request after the 20th of each month for the following month schedules.
